A towel bar that is not located in a good spot. This particular towel bar is located behind a toilet and you're, probably thinking eh that's better than not having a towel bar at all and you're. Maybe you're right, but once you place it behind the toilet, you've placed it behind an obstacle and you're asking for trouble and of course, I'm not about to suggest it's always going to happen.
But I think the biggest concern of mine would be young children who might be able to pull the towel down if it's hanging on the wall. But they would have a difficult time putting the towel back up on the towel bar. So they would need to put the toilet lid down, stand up on the toilet, lid and possibly the toilet tank depending upon how small they are so thinking about this.
If you're going to be placing a towel bar behind a toilet, if it's the only spot, go for it, if it isn't, then put it somewhere else, if you need to towel bars, maybe stack them putting one under the other or get a different type of towel. Bar system, you know, get some different hardware, but I don't like to see towel bars behind toilets because they are difficult to reach and they create obstacles for small children.
